Census Form
If you are new to Hastings or have a new addition to your family, please complete our district census form so that we can add or update the information we have for your family.  Completed census forms can be mailed to the district office or if you prefer, you can fax it to (651) 480-7014.

Denial of Release of Information Form (2018-2019)

Non-Household Emergency Contacts Form (for currently enrolled students ONLY)
Complete this form when changes/updates need to be made to the non-household emergency contact information that the school has on file for your student.

Student Health Information Form
To be filled out if a middle school or high school student has had a change in their health status.  Please fill out one form per student.  Completed forms should be returned to the school health office where the child attends.

Student Household / Address Change Form
To be filled out if a student has a change of address or a change in household information.  Please complete one form per student.  Completed forms should be returned to the school the student attends.​​
